Abstract :

The purpose of this study was to investigate the lived experiences of mothers of children with attention deficit/ hyperactivity disorder. The statistical population included all mothers of children with attention deficit/ hyperactivity disorder who referred to the psychiatric center of Fatemi Hospital in Ardabil city in 2023. The sample of the research enfolded 20 subjects whom were selected via purposive sampling method. The research method was a qualitative and phenomenological type. Data were attained through semi structured interviews with participant mothers of children with attention deficit/ hyperactivity disorder. Data were analyzed with the seven-step descriptive phenomenological method of Colaizzi (2002). The obtained results of mothers’ lived experience were classified in 5 main themes and 19 subthemes. The main themes involved child individual problems, family problems, social and emotional problems, medical issues and school problems. The findings resulted from the analysis of the lived experiences of the children with attention deficit/ hyperactivity disorder mothers also revealed the existence of a set of challenges including cultural diversity, culture-bound experiences and the participant educations levels. The other set of problems involved the mothers’ low knowledge and the educational environment. The children problems impose impairments on family natural function and influence their social, cultural and interactional relations. Therefore, making the mothers aware of the nature of the disorder and teaching interactive patterns with these children could prevent the problems facing these families.

Extended abstract

 

Introduction: Attention-deficit/ hyperactivity disorder is a neurodevelopmental disorder that starts from childhood and often continues until adulthood and with symptoms of inattention, hyperactivity and impulsivity, or a combination of hyperactivity, impulsivity, and inattention that is inconsistent with developmental level and disrupts daily functioning. Among the factors that are involved in the emergence of attention deficit/hyperactivity disorder, we can mention a combination of genetic and physiological characteristics and tendencies of children, family interaction patterns, the mutual influence of social and environmental conditions, which are among the most important factors. Attention deficit/hyperactivity disorder has a wide impact on the child, family, teachers and society and causes disruption and damage in functions such as academic success, behavior in school, interaction with family and communication with peers. All of these children have major functional problems in academic, family and social situations. The research results indicated the effects of this disorder on the family structure. For most families, this diagnosis is shocking and unexpected. So that some mothers deny the existence of a disorder in their child by rejecting the diagnosis and performing other tests and investigations. and experience more feelings of anger, anxiety and depression and have lower general health than the general population. Due to such circumstances the purpose of this study was to investigate the lived experiences of mothers of children with attention deficit/ hyperactivity disorder.

Conclusions: The results of the present study showed that attention deficit/hyperactivity disorder faces serious problems and challenges for the family and the child in various fields. The results of the present study indicated that mothers with children with attention deficit/ hyperactivity disorder face a series of problems and conflicts. Some of these problems were due to the low knowledge of mothers and the educational environment, and another was due to the exhausting nature of the disorder. Children's problems disrupt the normal functioning of the family and affect their social relations and interactions. Also, the main concern of these mothers was in the field of treatment and effectiveness of the drugs received. Many mothers did not have a correct attitude to the mechanism of action of the drug, so they considered its long-term use useless. In general, it could be said that the families of children with attention deficit/hyperactivity disorder were exposed to many problems due to the nature of the disorder, and it seemed very necessary to address these problems and provide the necessary solutions and training. Therefore, making the mothers aware of the nature of the disorder and teaching interactive patterns with these children could prevent the problems facing these families.

هدف پژوهش حاضر، بررسی تجربه زیسته مادران دارای کودک مبتلا به اختلال نقص توجه/ بیش­فعالی بود. جامعه آماری تمام مادران دارای  کودک مبتلا به اختلال نقص توجه/ بیش­فعالی بود که در سال 1401 به بخش روانپزشکی بیمارستان فاطمی شهر اردبیل مراجعه کرده بودند. نمونه پژوهش  20 نفر از  مادران بود که با استفاده از روش نمونه گیری هدفمند  انتخاب شدند. روش پژوهش کیفی از نوع پدیدارشناسی بود. داده‌ها با استفاده از مصاحبه نیمه ساختاریافته  با مشارکت مادران این کودکان گردآوری شد. داده‌ها به روش پدیدارشناسی توصیفی کلایزی (2002) تحلیل شد. یافته­های حاصل از تجارب زیسته مادران در قالب پنج مضمون اصلی و نوزده فرعی طبقه­بندی گردید. مضمون­های اصلی شامل مسائل فردی کودک، مسائل خانواده، مسائل اجتماعی، فرهنگی و هیجانی، مسائل درمانی و مسائل مدرسه بودند. هم­چنین نتایج به دست آمده از تحلیل تجارب زیسته مادران دارای کودک مبتلا به اختلال نقص توجه/ بیش­فعالی حاکی از وجود مجموعه­ای مشکلات از جمله تفاوت­های فرهنگی و تجربیات وابسته به فرهنگ و میزان تحصیلات شرکت­کنندگان اشاره کرد. دسته­ای  دیگر از این مشکلات به دلیل دانش کم مادران و محیط آموزشی است. مشکلات کودکان موجب اختلال در کارکرد طبیعی خانواده می­شود و روابط اجتماعی ، فرهنگی و تعاملات آن­ها را نیز تحت تاثیر قرار می­دهد. بنابرین آگاه سازی مادران از ماهیت اختلال و آموزش الگوهای تعاملی با این کودکان می تواند از مشکلات پیش رو در این خانواده­ها جلوگیری کند.

مقدمه

اختلال نقص توجه/ بیش­فعالی یک اختلال عصبی-تحولی است که از دوران کودکی شروع می­شود و اغلب تا بزرگسالی ادامه می­یابد (Singh & et al., 2022). نشانه­های این اختلال بی­توجهی، بیش­فعالی و تکانشگری و یا ترکیبی از بیش­فعالی، تکانشگری و بی­توجهی که با سطح رشد ناسازگار است و عملکرد روزانه را مختل می­کند، مشخص می­شود (Kasahara & et al., 2023). شیوع اختلال نقص توجه/ بیش­فعالی در کودکان 9/5 درصد و در بزرگسالان 5/2/1 درصد برآورد شده است (Faraone & et al., 2020). از عواملی که در به وجود آمدن اختلال نقص توجه/ بیش­فعالی دخالت دارند می­توان به ترکیبی از خصوصیات و گرایش­های ژنتیکی و فیزیولوژیکی کودکان، الگوهای تعامل خانوادگی، تأثیر متقابل شرایط اجتماعی، فرهنگی و محیطی اشاره نمود که از مهم­ترین عوامل در علت­شناسی اختلال نقص توجه/ بیش­فعالی، متغیرهای خانوادگی می­باشد (Speerforck & et al., 2019). خانواده نخستین و بادوام­ترین عاملی است که به عنوان سازنده و زیربنای شخصیت و رفتارهای بعدی کودک شناخته شده و حتی به عقیده بسیاری از روان­شناسان باید ریشه بسیاری از اختلالات شخصیت و بیماری­های روانی را در پرورش اولیه در خانواده جستجو کرد. والدین و به ویژه مادر اولین کسانی هستند که نوزاد با آنان رابطه برقرار می­کند لذا مهم­ترین نقش را در پرورش ویژگی­های روانی و عاطفی کودک به عهده دارند و کانون سلامتی یا بیماری محسوب می­شوند (Liang & et al., 2021). اختلال نقص توجه/ بیش­فعالي تأثیر گسترده­ای بر کودک، خانواده، معلمان و جامعه دارد و موجب بروز اختلال و آسیب در عملکردهایی نظیر موفقیت­های تحصیلی، رفتار در مدرسه، تعامل با خانواده و ارتباط با همسالان می­شود و در مجموع این کودکان مشکلات عمده کارکردی در زمینه­های تحصیلی، خانوادگی و موقعیت­های اجتماعی-فرهنگی دارند (Cortese & et al., 2015). رفتارهای بیش­فعالی-تکانشگری این کودکان باعث اختلال در روابط با گروه همسالان و طرد شدن آن­ها و هم­چنین منجر به مشکلات یادگیری و نتایج تحصیلی ضعیف می­شود (Robert & et al., 2021). وقتی چنین عواملی با تعارض­های درون خانوادگی با والدین و دیگر اعضای خانواده همراه می­شوند، موجب کاهش عزت نفس، خلق افسرده، تحمل کم برای ناکامی و کج خلقی می­شوند. اختلال نقص توجه/ بیش­فعالی سیر مزمن و تدریجی دارد، هم­چنین پیش آگهی این اختلال نامطلوب بوده و کودکان مبتلا در سال­های آینده در معرض خطر رشد مشکلات دیگری هم­چون اختلالات خلقی و اضطرابی، اختلال سلوک، مصرف مواد و نیز اختلال شخصیت ضد اجتماعی و رفتارهای بزهکارانه در دوران بلوغ و بزرگسالی هستند (Raiker, 2021).

نتایج پژوهش­ها حاکی از تاثیرات اختلال نقص توجه/ بیش­فعالی بر ساختار خانواده است. برای بیشتر خانواده­ها، این تشخیص تکان­دهنده و غیر منتظره است. به طوری که برخی والدین با رد تشخیص و انجام آزمایش­ها و بررسی­های دیگر، وجود اختلال در کودک خود را انکار می­کنند (Craig & et al., 2020). این فشارها می­تواند برخاسته از مشکلات ناسازگاری و رفتارهای ضد اجتماعی وفرهنگی، حرکات کلیشه­ای و یا فشارهای روانی ناشی از دشواری در برقراری ارتباط اجتماعی و نیز مشکلات مربوط به حضور در اماکن عمومی با شریط خاص جسمانی کودک و یا هزینه­های زیاد خدمات آموزشی و درمانی باشد (Risley & et al., 2020). نتایج پژوهش Si & et al (2020) نشان می­دهد که مادران کودکانی که اختلال برونی­سازی شده از جمله اختلال نقص توجه/ بیش­فعالی دارند؛ احساس موفقیت و اثربخشی کمتر و احساس خشم، اضطراب و افسردگی بیشتری را گزارش می­دهند و سلامت عمومی پایین­تری نسبت به جمعیت عمومی دارند. مشکلات روانی والدین از جمله سطوح بالای استرس و اختلال در کارکرد روانی اجتماعی آن­ها، می­تواند علت و پیامد مشکلات رفتاری این گروه از کودکان باشد (Zaslan & Yıldırım, 2021).

با توجه به آن­چه گزارش شد یکی از مسائل مهم خانواده­ها، داشتن فرزند دارای اختلال نقص توجه/ بیش­فعالی است. در حال حاضر این اختلال مسئله مهمی در بهداشت روانی جامعه محسوب می­شود و تشخیص آن از سال 1990 به طور چشمگیری افزایش یافته است (Nass & Leventhal, 2010) .اختلال نقص توجه/ بیش­فعالی هم کودک و هم والدین را تحت تأثیر قرار می‌دهد (Chronis-Tuscano & et al., 2010) . با توجه به این­که  این اختلال یکی از شایع­ترین اختلالات دوران کودکی است و با توجه به اثرات نامطلوب و پيامدهاي ناشي از آن، بررسی مشکلات پیش روی مادران و خانواده­ها در چنین شرایطی از اهمیت ویژه­ای برخوردار است. لذا هدف پژوهش حاضر، بررسی تجربه زیسته مادران دارای کودک مبتلا به اختلال نقص توجه/ بیش­فعالی می­باشد.

روش

طرح پژوهش، جامعه آماري و روش نمونه گيري

 

روش پژوهش کیفی است که با توجه به هدف پژوهش و دستیابی به تجربه زیسته مادران ، از روش پدیدارشناسی استفاده شد. پژوهش پدیدارشناسی پاسخ این پرسش را جستجو می‌کند که ساختار و ماهیت تجربه یک پدیده توسط انسان‌ها چیست؟ (Van Manen, 2016). در تحقیقات پدیدار شناختی کیفی، منظور از تجربه زیسته یک بازنمایی از تجربیات و انتخاب‌های یک شخص معین و دانشی است که او از این تجربیات و انتخاب‌ها به دست می‌آورد (Given, 2008). جامعه آماری شامل مادران دارای کودک مبتلا به اختلال نقص توجه/ بیش­فعالی شهر اردبیل در سال 1401 بود که به مرکز روانپزشکی بیمارستان فاطمی مراجعه کرده بودند. نمونه پژوهش 20 نفر از مادران بود که به روش نمونه گیری هدفمند انتخاب شدند.

 

روش اجرا

 

داده­ها با استفاده از مصاحبه‌های نیمه ساختاریافته جمع‌آوری شد که این فرآیند تا اشباع نظری داده­ها ادامه پیدا کرد. مدت زمان مصاحبه با هر یک از مادران حدود 40 دقیقه بود. پس از ضبط شنیداری، تمامی فایل­های صوتی حاصل از مصاحبه­ها، به رشته تحریر درآورده شد و به شیوه کدگذاری مورد تجزیه و تحلیل قرار گرفتند. ملاک­های ورود به پژوهش شامل: داشتن کودک 9-12 سال دارای اختلال نقص توجه/ بیش­فعالی، تشخیص اختلال توسط روانپزشک و رضایت آگاهانه جهت شرکت در پژوهش بود. وحق خروج از پژوهش در هر مرحله‌ای از پژوهش به مشارکت‌کنندگان داده شد. داده‌ها به روش پدیدارشناسی توصیفی کلایزی (1978) تحلیل شد. جهت اعتمادپذیری داده‌ها از چهار معیار جداگانه گوبا و لینکلن (Guba & Lincoln) استفاده شد.

 

ابزار سنجش

ابزار پژوهش در روش کیفی با روش کمی متفاوت است. در پژوهش کیفی پژوهش­گر به عنوان ابزار پژوهش قلمداد می­شود؛ زیرا در جریان مصاحبه به روش پرسشگری، یادداشت برداری و ضبط مصاحبه می­پردازد و در نهایت پاسخ­ها را تعبیر و تفسیر می­کند. بنابراین از دیدگاه روش­شناسان کیفی یکی از ابزارهای مهم جمع­آوری و تحلیل داده­ها در مطالعه کیفی پژوهشگر می­باشد.

 

 

يافته­ها

شرکت‌کنندگان در پژوهش مشتمل بر 20 مادر بود که گستره سنی آنان از 27 تا 45 سال با میانگین 5/34 سال بودند. از تجزیه و تحلیل داده­های بدست آمده از مصاحبه والدین 5 مضمون اصلی و 19 فرعی به دست آمد که خلاصه آن در جدول 1 گزارش شده است.

جهت توضیح بیشتر هر یک از مضامین اصلی، در ادامه نمونه ای از تجربه های مادران، به اختصار مورد بررسی قرار گرفتند.

-       مسائل کودک

یکی از مضامین استخراج شده، مسائل کودک است. مضامین فرعی عبارتند از: علائم اصلی اختلال، سابقه رشدی کودک و اختلالات همبود. شامل کدهای جنب و جوش و تحرک، پرحرفی، توجه و تمرکز ضعیف، حواس پرتی و فراموشکاری، صبر و تحمل پایین، بدون فکر رفتار کردن، حرف گوش نکردن، نبود نظم و انضباط، شکایت از غذا خوردن، استفاده بیش از حد از موبایل و تلویزیون، خواب نا منظم. در همین راستا یکی از مادران بیان کرد: "ساکن طبقه پنجم هستیم از پنجره آویزون میشه بیرون رو نگاه کنه. فقط موقع خواب یه جا بند میشه. به جز اون دائما در حال فعالیته. روی مبل بالا پایین میپره. حتی موقع نگاه کردن تلویزیون راه میره. خیلی صحبت می کنه. هر حرفی رو 100 بار تکرار می کنه. یه چیزی بهش میگم بعدش میگه یادم رفت". مادر دیگر: "موبایل و تلویزیون: همش گوشی دستشه بازی میکنه. جایی میریم به همه میگه برام اینترنت میخری بازی دانلود کنم؟ شده توی یه روز 600 تومن پول اینترنت برام اومده به خاطر بازیهاش. اصلا غذا نمیخوره. به زور قاشق رو میزارم دهنش. همش در حال راه رفتن بهش غذا میدم. همیشه با بشقاب غذا دنبالشم. همش هله هوله میخوره".

-       مسائل خانواده

     مضمون اصلی دیگر مسائل خانواده شامل مضامین فرعی سواد و دانش والدین، شیوه­های فرزندپروری، نگرانی­های والدین در مورد آینده کودک، روابط اعضای خانواده با کودک و تاثیر کودک دارای اختلال بر روابط اجتماعی و فرهنگی خانواده (چالش­های اجتماعی). کدهای استخراج شده شامل: تنبیه فیزیکی، توهین و تحقیر، قهر کردن، داد زدن، جایزه و پاداش دادن، تعریف و تحسین، اجرای منظم قوانین، همسویی هر دو والد و هم نظری در برخورد با کودک. احساس گناه، استرس و فشار عصبی، احساس بی کفایتی، خستگی و فرسودگی، صمیمیت کمتر، نبود وقت مشترک، دعوا و جر و بحث.  نرفتن به برخی موقعیت­ها و مهمانی ها، رفت و آمد محدود با فامیل، احساس شرم و خجالت، نگرانی از نگاه و قضاوت دیگران. مطالعه کتاب، کمک گرفتن از روان­شناس و مشاور. ارتباط با سایر مادران دارای کودک بیش فعال.  نگرانی در مورد آینده تحصیلی و شغلی. تشکیل خانواده، بهبودی و درمان در همین راستا یکی از مادران بیان کرد: " خیلی کتک میزنم. هر چی از دهنم درمیاد میگم بهش. میگم چه بچه بدی هستی. خاک تو سرت. دیگه مامانت نمیشم. با یه شلنگ نازک میزنمش بعد خودمم با اون شیلنگ میزنم. ولی میگم اگه مشقاتو بنویسی برات ماشین میخرم بهتر مینویسه. مادر دیگر: "نظم توی خونه ما صفره. همه چی رو زمینه. خیلی بی نظمه. وسایلاش همه جا پهنه. وقت خواب مشخصی نداریم هر وقت خودش بخواد می خوابه. حتی توی پول خرج کردن بی نظمیم. "

- مسائل اجتماعی، فرهنگی  و هیجانی

مضمون اصلی دیگر مسائل اجتماعی، فرهنگی و هیجانی شامل مضامین فرعی مشکلات هیجانی و تعاملات بین فردی. کدهای استخراج شده شامل: دوستان کم، پرخاشگری و زورگویی، فحش دادن، طرد شدن، تحریک پذیری، ترس، در همین راستا یکی از مادران بیان کرد: " توی کوچه درگیر میشه با بچه ها. یقه اونا رو میگیره. اصلا نمیتونه جلوی خودش رو بگیره". مادر دیگر: روابط عمومیش خوبه. با همه دوست می شه. خیلی سر و زبون داره. خدا رو شکر از این بابت خوبه". مادر دیگر: " خیلی بچه حساسیه. به خاطر شلوغ بودنش توی مدرسه دوستاش بهش میگن امیر دیونه خیلی ناراحت میشه میگه مامان اونجوری صدام میکنن خیلی ناراحت میشم. شبا تنها نمیخوابه.".

- مسائل مدرسه

مضمون اصلی دیگر مسائل مدرسه شامل مضامین فرعی انجام تکالیف، روابط با همکلاسی­ها و معلم، یادگیری و پیشرفت تحصیلی. کدهای استخراج شده شامل: بی علاقگی و به زور نوشتن تکالیف، بد خطی، گم کردن وسایل مدرسه، مشکل در توجه و تمرکز در کلاس، حواس پرتی زیاد در کلاس، رعایت نکردن نظم و قانون کلاس، بی اجازه بلند شدن، پذیرفته نشدن در گروه های مدرسه.  یکی از مادران بیان کرد: "معلمش میگه همش اجازه میگیره میره بیرون. همیشه کتاباش رو زمینه همه از روش رد میشن اصلا براش مهم نیست. حواسش تو کلاس نیست. معلمش تازگیا کارت تلاش میده برای کارای خوبش به خاطر گرفتن کارت تلاش خیلی به حرفای معلم گوش میده". مادر دیگر: "بعضی وقتا خوب و با سلیقه  مینویسه. سال تحصیلی تازه شروع شده تمام کتاباش پاره پوره است. باید دوباره کتاب براش تهیه کنم. هر روزم یه چیزی رو تو مدرسه جا میزاره. کلاهش رو جا گذاشته بود رفتم خریدم فرداش دوباره جا گذاشت تو مدرسه اومد".

- مسائل درمانی

   مضمون اصلی دیگر مسائل درمانی شامل مضامین فرعی مراجعه به روانپزشک و روان­شناس، میزان رضایت از درمان­های دریافتی دارو درمانی. سر وقت خوردن دارو و مقاومت در مصرف دارو. جلسات مشاوره. توانبخشی. کدهای استخراج شده شامل: رضایت از دارو، عوارض دارو، یکی از والدین بیان کرد: "ریتالین میخوره خیلی بهتر شده. تو مدرسه معلمش راضیه. توجه تمرکزش نسبت به قبل بهتره". مادر دیگر: " تیک پیدا کرده توی صورتش که خیلی نگرانم. دکتر گفته عوارض دارو هست به مرور از بین میره ولی این تیکش دیگه اعصابم و خورد کرده نمیتونم تحمل کنم."

بحث و نتیجه­گیری

نتایج پژوهش حاضر نشان داد اختلال نقص توجه/ بیش­فعالی خانواده و کودک را در حوزه­های مختلفی با مشکلات و چالش­های جدی روبرو می­کند. بررسی و تحلیل پاسخ­های مادران دارای کودک مبتلا به اختلال نقص توجه/ بیش­فعالی در قالب کدهای استخراج شده 5 مضمون اصلی را به دست آورد که عبارت­اند از: مسائل فردی کودک (شامل مضامین فرعی شاخص­های تحولی، اختلالات همبود، و علائم اصلی اختلال)، مسائل خانواده (شامل مضامین فرعی سواد و دانش والدین، شیوه­های فرزندپروری، نگرانی­های والدین در مورد آینده کودک، روابط اعضای خانواده با کودک و تاثیر کودک دارای اختلال بر روابط اجتماعی و فرهنگی خانواده (چالش­های اجتماعی) ، مسائل اجتماعی و هیجانی (شامل مضامین فرعی مشکلات هیجانی و تعاملات بین فردی)، مسائل درمانی (شامل مضامین فرعی مراجعه به روانپزشک و روان­شناس، میزان رضایت از درمان­های دریافتی) و مسائل مدرسه (شامل مضامین فرعی انجام تکالیف، روابط با همکلاسی­ها و معلم، یادگیری و پیشرفت تحصیلی) بود. 

  مضمون مسائل کودک به ویژگی‌های جسمی و روانی خاص کودک  اشاره دارد که می‌تواند ناشی از اختلال نقص توجه/ بیش­فعالی باشد و یا بر اختلال کودک تأثیرگذار باشد. این ویژگی‌های فردی می‌تواند خانواده را تحت تأثیر قرار دهد. مشکل حفظ توجه و نادیده گرفتن محرک­های مزاحم در طیف وسیعی از فعالیت­ها از ویژگی های اصلی این کودکان است (Huang-Pollock & et al., 2020). به دلیـل نقـص در توجـه و عــدم توانایــی در تمرکــز به راحتــی دچــار حواسپرتــی شــده و قـادر نیسـتند امـور خـود را براسـاس یک نظـم منطقی و تسلسـلی انجـام دهنـد (Young & et al., 2022). بسـیاری از متخصصان بـر این بـاور هستند که ناتوانــی در مهار رفتارهای تکانشگرانه، در اختلال نارسایی توجه/ بیش­فعالی نقـش بنیادی و اساسی را ایفا می­کند و با طیف وسیعی از پیامدهای نامطلوب همراه است (Young & et al., 2022). کودکانی که تکانشی هستند، معمولا قبل از انجام عمل فکر نمی­کنند. تکانشگری آن­ها اغلب به بیان نظرات نامناسب یا گرفتن اسباب باز ی از دیگران به دلیل مشکلاتی در منتظر نوبت ماندن منجر می­شود (Lukito & et al., 2020).

  مضمون دوم مسائل خانواده بود این مضمون شامل شیوه­های فرزندپروری، سواد و دانش والدین، روابط اعضای خانواده با کودک و تاثیر کودک دارای اختلال بر روابط اجتماعی خانواده و تعارضات والدین و کیفیت زندگی می­باشد. تولد کودک بیش­فعال عامل نگران کننده برای خانواده محسوب می­شود (Corona, 2020). به طور کلی شواهد حاکی از آن است که اختلال نقص توجه/ بیش­فعالی با آشفتگی­های خانوادگی و زناشویی، روابط ناکارآمد والد- کودک، شناخت نداشتن والدین از رفتار کودک، خودکارآمدی ضعیف در فرزندپروری، تنیدگی فرزندپروری بالا و آسیب روانی والدین همراه است (Haack & et al., 2016). والدین کودکان دارای اختلال نقص توجه/ بیش­فعالی به ویژه مادران، اغلب به گونه­ای زیان­آور، سطح بالایی از فشار روانی را تجربه می­کنند و سطوح بالای استرس­های والدینی در تعامل والد- کودک به مهارت­های فرزندپروری والدین آسیب می­رساند (Afshar & et al., 2022). احساس تقصیر و گناه، احساس ناکامی و اضطراب و نگرانی، از جمله واکنش­های شایع مادران در پی آگاهی از بیماری کودک است (Anker & et al., 2020). بسیاری از خانواده­ها دانش چنداني نسبت به وضعیت خاص رواني و رفتاری کودک با اختلال رفتاری ندارند و همین بی اطلاعی باعث درگیری بیشتر والدین مي­شود و موجب اجراي شیوه­هاي فرزندپروري نامطلوب می­شود. این والدین بیشتر از والدین کودکان عادی، از شیوه­های فرزندپروری مستبدانه و شیوه­های تنبیهی استفاده می­کنند (Halomy, 2011) . البته شیوه­های فرزندپروری ناکارآمد آن­ها، در رشد و حفظ مشکلات رفتاری کودکان دارای اختلال نقص توجه/ بیش­فعالی نقش مهمی دارند (Nori & et al., 2023).

  در چنین خانواده­هایی، کودکان دارای اختلال، معمولا مورد بی­مهری، بی­تفاوتی از سوی حداقل یکی از والدین یا حتی تنبیه بدنی و ضرب و شتم به اتهام عدم فرمان­پذیری، گستاخی و اخلاگری قرار می­گیرند و مشکلات و تبعات منفی این موارد به­طور مرتب زندگی خانوادگی و رضایت زوجین و سلامت روان مادران را متأثر می­کند. این مادران در پی موقعیتی هستند که فارغ از مشکلات رفتاری کودک ­شان بتوانند در مهمانی­ها شرکت کنند، به خرید بروند و از ارتباطات اجتماعی خود احساس خوشایندی داشته باشند (Torabi & et al., 2016). اما به دلیل نشانگان اختلال کودک، رفتارها و واکنش­های خارج از عرف فرهنگ، عدم اطلاع بستگان، برای رهایی از انتقاد و سرزنش سعی در محدود کردن روابط خود دارند (Eftari & et al., 2022).

 

مضمون سوم­ مسائل اجتماعی ، فرهنگی و هیجانی است. این مضمون اشاره به مشکلات هیجانی و تعاملات بین فردی کودک دارد. مشکلات اجتماعی، فرهنگی و هیجانی یکی از مهمترین حیطه­هایی است که کودکان و نوجوانان با اختلال نقص توجه/ بیش­فعالی در آن دچار آسیب هستند (Yep & et al., 2018). این اختلال با نقص در روابط بین­فردی و پذیرش پایین از طرف همسالان ارتباط دارد (Kim & et al., 2015). کودکان مبتلا اغلب اوقات عصبانیت، پرخاشگری و تهاجم یا گوشه­گیری و افسردگی دارند (Wolraich & et al., 2019). مشکل در مدیریت احساسات منفی، مانند خشم و احساسات مثبت، مانند هیجان بیش از حد منجر به پیامدهای اجتماعی،فرهنگی و عملکردی ضعیف می­شود (Yue & et al., 2022). و آسیب جدی در روابط بین فردی ایجاد می­کند.  Christian & et al (2020)بر اساس نظریة ناکارآمدی بازداری مطرح می­کند که کودکان دارای اختلال نقص توجه/ بیش­فعالی نمی­توانند به طور مؤثر به محیط پاسخ تأخیری بدهند و هیجان­های متفاوتی را در یک محیط اجتماعی و فرهنگی نشان می­دهند که متناسب با آن محیط نیست، زیرا نمی­توانند به نشانه­های ضروری اجتماعی وفرهنگی در رفتارهایشان ی که غیرمتناسب هستند، توجه کنند. این افراد در تعدیل شدت هیجانات، تکانشگری هیجانی، کنترل واکنش­های هیجانی منفی دارای مشکلاتی هستند و الگوهای تنظیم هیجان ناکارآمدی دارند (Moradi & et al., 2022).

مضمون چهارم مسائل مدرسه است. این مضمون اشاره به نحوه انجام تکالیف، روابط با همکلاسی­ها و معلم، یادگیری و پیشرفت تحصیلی و چالش­های والدین در حوزه تحصیل کودک دارد. کودکان با اختلال نقص توجه/ بیش­فعالی اغلب با مشکلات عملکرد تحصیلى مواجه هستند و موفقیت و پیشرفت تحصیلى کمی دارند (Thapar & Cooper, 2016). از جمله مشکلاتی در زمینه فعالیت تحصیلی مانند فراموشی تکالیف خانگی، تکمیل پروژه­های بلندمدت، مطالعه برای امتحان و سازماندهی وسایل و لوازم شخصی و رفتارهايي چون دشواري در توجه، گوش كردن به دستورات و تمركز بر آن، خسته شدن در انجام كار در ظرف چند دقيقه، دست خط بد، ناهماهنگي و بي­برنامگي در حركت، دشواري و ناتواني در يادگيري و گاهي خيالبافي دارند (Brown & Whiteside, 2017). بسياري از اين دانش­آموزان توسط همسالان طرد مي­شوند كه اين مساله زمينه آسيب­هاي جدي رواني در آن­ها را افزايش مي­دهد (Gaastra & et al, 2020). علاوه بر همکلاسی­ها، روابط با معلمان نیز تحت تاثیر قرار می­گیرد.  در پژوهشی به بررسی كيفيت رابطه معلم با دانش­آموزان بيش فعال و عادي پرداختند. نتايج اين پژوهش حاكي از آن بود كه در ارتباط عاطفي و تحصيلي معلم با دانش­آموزان بيش­فعال و عادي تفاوت معناداري وجود دارد 2020) Zendarski & et al.,).

مضمون پنجم مسائل درمانی است. این مضمون درباره مداخلات درمانی و میزان رضایت از درمان­های دریافتی دارد. پروتکل­های مداخله درمانی بیش­فعالی، ترکیبی متشکل از مداخله­هایی نظیر دارو درمانی، آموزش خانواده، مدیریت رفتاری، توانمندسازی در کارکردهای اجرایی و حمایت­های اجتماعی، فرهنگی و روان­شناختی است  (Yavari Barhaghtalab & et al., 2021).

مداخله دارویی می­تواند از طریق کاهش علائم اصلی به­صورت غیرمستقیم کارکرد اجتماعی  و فرهنگی را نیز بهبود بخشد. علاوه بر این برخی کودکان و والدین آن­ها ممکن است تمایلی به مصرف داروهای محرك نداشته باشند (Sakhaei & et al., 2022). در این شرایط توجه به درمان­های دیگری که در کاهش علائم موثر است، برجسته می­شود (Davidson, 2008). شواهد نشان می­دهد استفاده همزمان از دارو و رفتاردرمانی بهترین نتیجه ممکن را در پی خواهد داشت (Sakhaei & et al., 2022). هم­چنین در میان مداخله­های غیردارویی ، توانبخشی شناختی نیز یکی از متدهایی است که امروزه مورد توجه متخصصان و درمانگران بالینی قرار گرفته است. با توانبخشی و به کارگیری روش­های روان درمانی، این گونه افراد را با شرایط محیطی می­توان سازگارتر کرد. بنابراین برای افراد مختلف حالت­های مختلف و به تناسب آن درمان­های متفاوت را باید به کار گرفت (Bayat & et al., 2018).

 به طور کلی نتایج پژوهش حاضر حاکی از این است که مادران دارای کودک مبتلا به اختلال نقص توجه/ بیش­فعالی با مجموعه­ای از مشکلات و تعارضات روبه­رو هستند. از جمله می­توان به تفاوت­های فرهنگی و تجربیات وابسته به فرهنگ و میزان تحصیلات شرکت­کنندگان اشاره کرد. دسته­ای  دیگر از این مشکلات به دلیل دانش کم مادران و محیط آموزشی است. مشکلات کودکان موجب اختلال در کارکرد طبیعی خانواده می­شود و روابط اجتماعی ، فرهنگی و تعاملات آن­ها را نیز تحت تاثیر قرار ­دهد. در مجموع می­توان گفت خانواده کودکان دارای اختلال نقص توجه/ بیش­فعالی به دلیل ماهیت اختلال در معرض مشکلات متعددی می­باشند که پرداختن به این مشکلات و ارائه راهکارها و آموزش­های لازم بسیار ضروری به نظر می­رسد.
