چکیده
زمینه و هدف: بیماری کلیوی مرحله پایانی با عوارض گسترده جسمی، روانی، اجتماعی و اقتصادی همراه است و کیفیت زندگی بیماران از جمله کیفیت خواب آنان را تحت تأثیر قرار میدهد. این مطالعه با هدف پیشبینی کیفیت خواب بر اساس روشهای مقابله و خودکارآمدی با نقش میانجیگری تنظیم هیجان در بیماران مبتلا به بیماری نارسایی مزمن کلیه انجام شد.
روش بررسی: پژوهش حاضر از نوع توصیفی ـ همبستگی و با رویکرد مدلسازی معادلات ساختاری بود. جامعه پژوهش بیماران مبتلا به بیماری نارسایی مزمن کلیه شهر تهران در سالهای ۱۴۰۲ تا ۱۴۰۳ بودند. نمونهای به حجم ۲۳۷ نفر از سه بیمارستان (ملاصدرا، امیریه و بعثت) به روش در دسترس انتخاب شدند. دادهها با پرسشنامههای استاندارد که شامل پرسشنامه کیفیت خواب پیتزبورگ (1989) ، پرسشنامه راهبردهای مقابله ای لازاروس و فولکمن (1980) ، پرسشنامه خودکارآمدی عمومی شرر (1982) و پرسشنامه راهبردهای تنظیم هیجان گراس (2003) بود و با نرمافزارهای SPSS-20 و Smart PLS3 تحلیل شدند.
یافتهها: بیشتر شرکتکنندگان مرد (۶۴٫۱٪) و بالای ۵۰ سال (۶۹٫۲٪) بودند. روشهای مقابله (β=۰٫۳۸۴) و خودکارآمدی (β=۰٫۱۸۵) اثر مستقیم و معناداری بر کیفیت خواب داشتند (p<۰٫۰۵). همچنین، تنظیم هیجان نقش میانجی معناداری در رابطه روشهای مقابله و کیفیت خواب (β=۰٫۰۷۶) و خودکارآمدی و کیفیت خواب (β=۰٫۰۵۲) ایفا کرد (p<۰٫۰۵).
نتیجهگیری: نتایج پژوهش حاضر نشان داد که خودکارآمدی با میانجیگری تنظیم هیجان میتواند کیفیت خواب بیماران مبتلا به بیماری نارسایی مزمن کلیه را بهطور معناداری پیشبینی کند. این یافته بیانگر آن است که بیماران با سطح بالاتر خودکارآمدی، از مهارتهای تنظیم هیجان مؤثرتری برخوردارند و در نتیجه خواب باکیفیتتری تجربه میکنند. ازاینرو، ارتقای خودکارآمدی و آموزش راهبردهای تنظیم هیجان میتواند بهعنوان رویکردی مؤثر در بهبود کیفیت خواب و سازگاری روانی بیماران مبتلا به بیماری نارسایی مزمن کلیه مدنظر قرار گیرد.

Abstract
Background and purpose: End-stage renal disease is associated with extensive physical, psychological, social, and economic complications and affects the quality of life of patients, including their sleep quality. This study aimed to predict sleep quality based on coping methods and self-efficacy with the mediating role of emotion regulation in patients with renal failure.
Method of examination: The present study was descriptive-correlational and used a structural equation modeling approach. The research population was patients with renal failure in Tehran in 2013-2014. A sample of 237 people from three hospitals (Molla Sadra, Amirieh, and Besat) was selected by convenience sampling. The data were collected using standard questionnaires, including the Pittsburgh Sleep Quality Questionnaire (1989), the Lazarus and Folkman Coping Strategies Questionnaire (1980), the Scherrer General Self-Efficacy Questionnaire (1982), and the Gross Emotion Regulation Strategies Questionnaire (2003), and were analyzed using SPSS-20 and Smart PLS3 software.
Findings: Most of the participants were male (64.1%) and over 50 years of age (69.2%). Coping methods (β=0.384) and self-efficacy (β=0.185) had a direct and significant effect on sleep quality (p<0.05). Also, emotion regulation played a significant mediating role in the relationship between coping strategies and sleep quality (β=0.076) and self-efficacy and sleep quality (β=0.052) (p<0.05).
Conclusion: The results of the present study showed that self-efficacy, mediated by emotion regulation, can significantly predict sleep quality in patients with chronic renal failure. This finding indicates that patients with higher levels of self-efficacy have more effective emotion regulation skills and, as a result, experience better quality sleep. Therefore, promoting self-efficacy and training emotion regulation strategies can be considered as an effective approach to improving sleep quality and psychological adjustment in patients with renal failure.
